What Is the MILD® Procedure?

MILD® addresses a primary cause of LSS: the thickening of the ligamentum flavum, which narrows the spinal canal and compresses nerves. During the procedure, specialized tools are used to remove small portions of this ligament, decompressing the spinal canal and alleviating symptoms like pain, numbness, and limited mobility.

Performed through a single, tiny incision (smaller than a baby aspirin), MILD® is a safe alternative for patients seeking relief without the risks associated with open surgery.

How It Works

The MILD® procedure is typically completed in under an hour and involves:

Local Anesthesia – Numbs the treatment area; light sedation may be used.

Small Incision – A tiny incision is made in the lower back.

✔ Tissue Removal – Using imaging guidance, excess ligament tissue is removed to decompress the spinal canal.

✔ No Implants – The procedure leaves no implants behind; only a small bandage is needed.

Most patients return home the same day and resume normal activities within 24 hours.

Conditions Treated with MILD®

MILD® is effective for patients experiencing:

Lumbar Spinal Stenosis

Narrowing of the spinal canal causing nerve compression.

Neurogenic Claudication

Pain or numbness in the legs when standing or walking, relieved by sitting or bending forward.

Chronic Lower Back Pain

Persistent pain due to spinal canal narrowing.

Degenerative Disc Disease

Age-related wear and tear on spinal discs leading to stenosis.

Benefits of the MILD® Procedure

Patients undergoing the MILD® procedure can expect:

Minimally Invasive

Tiny incision with minimal tissue disruption, leading to reduced pain and faster healing.

Quick Recovery

Most patients resume normal activities within 24 hours without extended downtime.

Preserves Future Options

Keeps other treatment options available if symptoms return or progress.

No General Anesthesia

Performed under local anesthesia, reducing risks and recovery time significantly.

No Implants or Stitches

Leaves no implants; only a small bandage is needed for wound care.

Covered by Medicare

Nationwide coverage under Medicare and Medicare Advantage plans is available.

What to Expect During the Procedure

01 Before the Procedure

✔ Your provider will review your medical history, symptoms, imaging results, and any medications or allergies.

✔ You’ll receive fasting instructions (no food or drink for several hours before).

✔ You’ll be advised to arrange a ride home, as driving after the procedure is not recommended.

Any questions or concerns will be addressed before scheduling the procedure.

02 During the Procedure

✔ Local anesthesia will numb the treatment area no general anesthesia is needed.

✔ A specialized needle is used to create a small incision in your lower back.

✔ X-ray guidance ensures accurate access to the compressed spinal area.

✔ Excess bone and ligament tissue are removed to relieve pressure on spinal nerves.

✔ A small bandage is placed to close the incision no stitches required.

03 After the Procedure

✔ You’ll be monitored briefly and discharged the same day.

✔ You’ll need someone to drive you home.

✔ Most patients return to light activities within a few days.

✔ Full recovery may take a few weeks depending on your condition.

Follow your doctor’s instructions, including avoiding strenuous activity and attending follow-up visits.

Ideal Candidates for MILD®

You may be a suitable candidate for the MILD® procedure if:

You experience pain or numbness in the lower back or legs when standing or walking.

Symptoms improve when sitting or bending forward.

Conservative treatments (e.g., physical therapy, medications) have not provided relief.

You seek a minimally invasive alternative to open spine surgery.

Patients with prior lumbar spine surgeries should consult their healthcare provider to determine eligibility.

Risks and Potential Side Effects

While MILD® is generally safe, potential risks include:

Common Side Effects

Mild soreness or bruising at the incision site, typically resolving without intervention.

Rare but Serious Complications

Infection, bleeding, dural tear (cerebrospinal fluid leak), or nerve injury. Your provider will discuss all potential risks and benefits prior to the procedure.
